Cancellation Policy: 

A 24-hour notice of cancellation is required, otherwise; you will be charged  half of the fee.
Please note insurance companies will not reimburse this fee.

FEES:

Assessment - $150

Individual     - $100

Couple or Family - $100

I accept cash or check only.

Payment:

I am an "in-network provider" for BCBS.  If you are insured with BCBS, I will file the claim for you.  You will be responsible for any portion BCBS does not pay, within one week, after BCBS has processed the claim.

If you are a student and are covered by student health insurance ONLY, I am willing to file your claim.  If student health insurance is your secondary insurance, I will give you the

necessary paperwork for you to file the claim.

If you are insured with any other company, it is my preference that you pay for your session, at the time of your appointment.  I will complete the insurance claim form, which you would send to your insurance company and they will reimburse you, according to your plan.  If this is a problem for you, I am willing to discuss other options, including a payment plan.

 

Insurance:

The best way to find out, if you have mental health coverage is to call your insurance company.  It is advisable to do this before your first therapy appointment.  The telephone number is on the back of your medical card.  The following questions are pertinent:

• Do I have Behavioral Health coverage?
• How many sessions are covered?
• Do I have a deductible?
• Has any of my deductible been met?
• Do I have a co-payment or co-insurance amount?
• Is pre-authorization required?
